Het kwaliteitsbeleid in de Vlaamse ziekenhuizen beweegt volop na de eerdere accrediteringsgolven die zich hoofdzakelijk toespitsten op externe controles van ofwel Joint Commission International<\/em> (JCI) ofwel Qualicor Europe (vroeger Niaz). In het nieuwe kwaliteitscontrolelandschap vormen 19 instellingen samen een consortium dat ondersteund wordt door het Leuvens Instituut Gezondheidszorgbeleid (LIGB). Dat ligt aan de basis van<\/strong> FlaQuM 2.0. FlaQuM staat voor Flanders Quality Model<\/em> en wil internationaal hoge ogen gooien.<\/strong><\/h4>\n\n\n\n

Het LIGB geeft tekst en uitleg bij FlaQuM 2.0, dat verder ontwikkeld is samen met de 19 koploperorganisaties. Een brochure daarover werd onlangs in de ziekenhuizen verspreid. Nieuwe organisaties die wensen deel te nemen aan het consortium zijn welkom vanaf januari 2023.<\/p>\n\n\n\n

Twee essenti\u00eble zaken beklemtoont het LIGB bij monde van de directeur kwaliteit UZ Leuven, prof. dr. Dirk De Ridder. Ten eerste: FlaQuM 2.0 is geen afvinklijstje van het type JCI of Qualicor om aanspraak te maken op vijf sterren. Het gaat eerder om een kwaliteitsmanagement- en beheersysteem dat je moet laten dragen door je medewerkers, waarvoor methodologie\u00ebn worden aangereikt. Zaken waar men absoluut niet op wil afdingen, zijn er natuurlijk wel: denk aan de safe surgery checklist<\/em> bijvoorbeeld. En de harde indicatoren die je ziekenhuis kunnen positioneren.<\/p>\n\n\n\n

Ten tweede: FlaQuM 2.0 ontstond vanaf de werkvloer, met een belangrijke mate van medezeggenschap van de ziekenhuizen. \u201cDat is redelijk uniek in de wereld. Bovendien wordt dit project gepubliceerd in de internationale literatuur. Een van de meest transparante manieren om een kwaliteitssysteem op te bouwen is dit dus. De tweede golf van ziekenhuizen die aansluit vanaf 2023 zal het wat makkelijker hebben, maar ze zal wel de filosofie en de cocreatie nog in de vingers moeten krijgen, die is ontwikkeld door de eerste golf van FlaQuM-leden.\u201d<\/p>\n\n\n\n

\u201cMet de indicatoren kunnen we ziekenhuizen onder andere benchmarken op MZG-basis. (minimale ziekenhuisgegevens). \u201cAls in sommige ziekenhuizen de kans op pneumonie vier keer hoger ligt, dan is dat moeilijk extern te verklaren. Idem dito voor sepsis waarvan de outcome<\/em> tussen ziekenhuizen erg kan verschillen. Vaak liggen structurele problemen aan de basis.\u201d<\/p>\n\n\n\n

Prof. dr. De Ridder verwijst naar zijn eigen vakgebied, urologie: \u201cAls de bottom-performance<\/em> het evengoed zou doen als het mediane ziekenhuis, dan sparen we 432 urologische levens per jaar in Belgi\u00eb, of bijna een airbus per jaar!\u201d<\/p>\n\n\n\n

\u201cVoor oncologie doen de oncologische diagnostic related groups<\/em> (DRG\u2019s) het duidelijk beter dan de niet-oncologische. Waarschijnlijk wegens de multidisciplinair oncologisch consulten, het Kankerplan, een stuk meer guidelines<\/em>, meer peer review<\/em> in oncologie\u2026 Neem pyelonefritis, een ontsteking van nierweefsel. De behandeling kan erg uiteenlopen afhankelijk van wie je opvangt op spoed. De mortaliteit in die DRG\u2019s is veel variabeler in vergelijking met oncologische DRG\u2019s. Dat zijn harde, soms confronterende cijfers.\u201d<\/p>\n\n\n\n

Volgens Kris Vanhaecht, professor LIGB,zijn de ziekenhuizen vragende partij voor een blijvende en zelfs sterkere inspectie, maar is continu overleg met de sector broodnodig. \u201cDe ziekenhuizen geven zelfs aan dat onaangekondigde inspecties nodig zijn en dat de overheid haar rol blijft spelen, ook in de toekomst. Dat betekent niet dat we daarmee alle gaten zullen kunnen dichten.\u201d<\/p>\n\n\n\n

FlaQuM begint ook internationaal hoge ogen te gooien. Geregeld is er overleg met een groep internationale experts en ook de Waalse collega\u2019s van het Plateforme pour l\u2019Am\u00e9lioration continue de la Qualit\u00e9 des soins et de la S\u00e9curit\u00e9 des patients<\/em> (PAQS) zijn er vertegenwoordigd. Prof. dr. De Ridder: \u201cDe Franstalige collega\u2019s staan in contact met ons om te bekijken wat ze van ons en van hen kunnen overnemen.\u201d Professor De Ridder zelf was te gast bij de direction de qualit\u00e9 de l\u2019assistance publique de Paris<\/em> die de Centres Hospitaliers Universitaires<\/em> (CHU\u2019s) van Frankrijk verzamelt en daar stelde hij het FlaQuM-model voor.<\/p>\n\n\n\n
